No medical jargon. Just what we do, when we do it, and how to reach us.

If your loved one is in a nursing home long-term

  • How often: A doctor or nurse practitioner sees them at least once a month — more often if their health changes.
  • Communication: Expect a call from us when something significant changes. And you can call us any time.
  • Decisions: If you have power of attorney, we follow it. If your family has wishes about hospice or comfort care, we listen first. We will never make a major decision without you.
  • After hours: Call the same number. Our on-call doctor calls you back — day, night, weekends.
  • Visiting today? Call us and we'll tell you when the doctor is next at the facility — you're welcome to be there.

If your loved one is in short-term rehab

  • First visit: Within 2 to 3 days of arriving at the facility.
  • Care plan meeting: Sometime in the first week — you're invited.
  • Going home: We help plan the transition and make sure their family doctor gets a summary.

How we talk to you

We call. We email. We sit down for family meetings when the plan changes. If you have a question, you should be able to get a real person — usually within 20 minutes after hours.
