If your loved one is in long-term care

  • How often: A doctor or NP sees them at least once a month, more often if their health changes.
  • Communication: Expect a call from us when something significant changes — and you can call us any time.
  • Decisions: We respect Power of Attorney and family preferences. We will never make a major decision without you.
  • After hours: Our line reaches a real on-call provider day, night, weekends.

If your loved one is in short-term rehab (post-acute)

  • First visit: Within 48–72 hours of arriving at the facility.
  • Care plan: A meeting in the first week — you're invited.
  • Going home: We help plan the transition and make sure your PCP gets a summary.

How we communicate with you

We call. We email. We sit down for family meetings when the plan changes. If you have a question, you should be able to get a real person in a reasonable time.
